I am voluntarily joining the Healthy Habits Challenge or Healthy Habits Challenge PLUS sponsored by Fitness Pros Plus, LLC.
I understand that I should seek the advice of my physician before beginning an exercise program. Depending on an individual’s health status, there may be risks associated with this health and wellness program. If I have questions about the Healthy Habits Challenge and it’s appropriateness for me, I should likewise seek the counsel of my physician. Participation in the Healthy Habits Challenge should not override or take the place of medical advice given by my physician. Fitness Pros Plus makes no claim that this program is appropriate for all individuals.
Exercise, health and wellness, nutrition and motivational tips provided by Fitness Pros Plus, LLC are for informational purposes only, and if I have questions or concerns about the information provided, I should seek the advice of my physician or a Registered Dietician before relying on them for personal application.
I may stop participation in the Healthy Habits Challenge at any time.
The $21 Registration fee collected by Fitness Pros Plus, LLC, is non-refundable. I understand that $15 of the fee will be put into a contest pool and will be distributed to the winner of the contest at the end of the 8 weeks. Fitness Pros Plus will record the data, and determine the winner of the contest. If there is more than one winner (a tie) Fitness Pros Plus will distribute the pool evenly among the winners. Winnings will be distributed within one week of the end of the contest.
Having been informed of the procedures and the potential risks involved, I do knowingly and willfully assume any and all risks, known and unknown, that may result from my participation in this program.
In consideration of permitting me to participate in the Healthy Habits Challenge program, I knowingly and willingly release and forever discharge Fitness Pros Plus, LLC from all liability for all injuries and losses, whether foreseeable or unforeseeable, that I may suffer as a direct or indirect result of my participation.
I further state that I am legally competent to sign this release, that I understand these terms are contractual and not mere recital, and that I have signed this document as my own free act. The terms of this agreement shall serve as a release binding on my heirs, assigns, personal representatives, and on all members of my family, including any minors.
